The CEAT Student Council would like to know the status of students in the College who are affected of the recent Mt. Bulusan Eruption in Sorsogon this June 5, 2022. In line with that, we request those affected students to answer this form:
All personal/sensitive information will be kept confidential. By answering this form, you hereby give consent to the owners to store and use necessary information for the aforementioned cause or event in accordance with RA 10173 (also known as the Data Privacy Act of 2012).
